A Censuswide survey published by IBM on October 14 found that 80% of EMEA respondents said an AI incident had reached their board or C-suite in the previous year. Among the organizations reporting an incident, 40% said they changed AI governance policy or frameworks in response.

The survey covered 3,221 senior executives at organizations with at least 1,000 employees. Fieldwork ran September 14–24 in the UK, France, Germany, Italy, Spain, the Netherlands, Saudi Arabia and the UAE. These are responses to a survey conducted for IBM, not an independently verified incident count.

## Data

### Survey findings

IBM-reported EMEA survey responses; percentages are respondent shares, not independently audited incident statistics.

Methodology: Censuswide surveyed 3,221 senior executives at organizations with at least 1,000 employees in the UK, France, Germany, Italy, Spain, the Netherlands, Saudi Arabia and UAE, September 14–24, 2026. Percentages are IBM-reported shares; EMEA is the source-defined eight-country sample. The total Censuswide survey sample was 3,221 executives. The 40% denominator is organizations reporting an incident; the 80% denominator is all survey respondents. The exact subgroup size is not stated, so no headcount is derived.

| Finding | Share of respondents | Population / basis |
|---|---|---|
| AI incident reached board or C-suite in previous year | 80 | Respondents in the source-defined eight-country EMEA sample |
| Changed governance after an AI incident | 40 | Organizations reporting an AI incident; exact subgroup size not stated |
